Why children's BMI needs percentile charts instead of adult cutoffs, and how the eGFR kidney function formula works.

Pediatric BMI and kidney function (eGFR) are both calculated health metrics that rely on more complex interpretation than their adult counterparts — one because bodies change dramatically with age, the other because the underlying formula itself has several moving parts.

Pediatric BMI: same formula, different interpretation

The BMI formula itself doesn't change for children — weight(kg) / height(m)² — but the interpretation does. Adult BMI uses fixed cutoffs (18.5, 25, 30) regardless of age; pediatric BMI must instead be plotted against age- and sex-specific percentile growth charts, since a healthy body fat percentage for a 6-year-old is very different from a healthy one for a 16-year-old, even at an identical raw BMI number.

Why percentile, not raw BMI, is the clinically useful number

A child's BMI percentile shows where they rank compared to other children of the same age and sex, based on large reference population data (commonly CDC or WHO growth charts). A BMI that would be "normal" by adult standards could actually represent an unusually high or low percentile for a specific child's age — which is exactly why pediatricians track percentile trends over time rather than a single raw BMI value.

eGFR: estimating kidney filtration rate from blood creatinine

The 2021 CKD-EPI equation estimates glomerular filtration rate from serum creatinine, age, and sex, using several empirically-derived constants (denoted kappa and alpha) that differ between men and women. The formula's complexity — multiple exponent terms and sex-specific constants — reflects that it was built by fitting to a large clinical dataset relating creatinine to more precisely measured filtration rates, not derived from first-principles physiology.

Why eGFR values are interpreted as ranges, not exact numbers

An eGFR above 90 mL/min/1.73m² is generally considered normal kidney function; below 60 for three months or more may indicate chronic kidney disease. But eGFR naturally fluctuates with hydration, recent exercise, and muscle mass (since creatinine is a muscle metabolism byproduct) — clinicians typically look at trends across multiple readings rather than reacting to a single value.

Common mistakes to avoid

  • Applying adult BMI category cutoffs directly to a child's BMI number instead of using an age/sex-specific percentile chart
  • Treating a single eGFR reading as a definitive kidney health diagnosis rather than one data point requiring clinical context
  • Forgetting that both of these formulas were derived from specific reference populations and can be less accurate outside those populations' typical characteristics
